Requesting a Certificate of Insurance

The purpose of a certificate of insurance is to demonstrate proof of insurance coverage to the parties involved.

All requests for a certificate of insurance should be processed through the office of Risk Management.

Request Procedure

Fill out the Certificate of Insurance (COI) request form below.

Submit the completed form/documentation via email.
