    Southeast Parent and Guardian Note Form

    This form will take approximately 4 minutes to complete. The purpose of this form is to allow parents the ability to submit parent notes for any absences that their child may have. Each student is allowed ONLY ten (10) DAYS OF PARENT NOTES per academic school year (August 16, 2023 through May 31, 2024).


    All Parent Notes must be received within 48 hours of the student's return to school. If an excuse is not received in the time required, the absence will be considered unexcused.

    In the event that your child has received medical attention and has seen a medical doctor you may submit a medical excuse from the medical provider, hospital or clinic, with proper signatures affixed thereto, by uploading and sending excuses to the Registrar, Paris Stewart.
