Dear Parents, 

The Richland County School District One Athletic Department would like to thank you for taking the time to complete the South Carolina High School League required forms to participate in the athletic program. All student athletes 7-12th grade are required to fill out and submit these forms online - PlanetHS. 

Richland County School District One and the SCHSL require the parent/legal guardian to complete the following electronic forms. The Physical and Medical History, must be completed by the physician, and the Concussion and Parent Permission form.  Other forms for parents include the Heat information sheet and student athletic handbook.  Students that will participate in athletics each school year are required to complete all forms and upload to PlanetHS. Parents and students are required to read the forms as they pertain to them. This year’s paperwork can be completed online to make the process convenient for all families. 

The physical must be dated after April 1 of the corresponding year. 

The South Carolina High School League and Richland County School District One requires an annual physical.
