Screening Process

Click here on OT/PT Referral Form to print and submit a referral to screen a student for services, and send it to the OT/PT office at Fairwold school attention Karen Jenny. 

Upon request of the OT/PT referral, be sure to obtain parent permission to screen prior to submitting to the OT/PT office. After the referral is made and a parent consent has been received, the therapist will screen the student by observation, consultation with the teacher and/or referring source, and consultation with the parent. When an evaluation is indicated, a parent consent is requested followed by a doctor's referral. Both consents must be in place prior to a formal evaluation of the student for therapy services.

After the evaluation is performed, the parent and teacher are notified of the results. The student then, if deemed appropriate for therapy services, will receive services to assist the educational process.
